IDENTIFICATION - Driver's License, DMV ID card, Alien Registration Card
CITIZENSHIP STATUS - Birth Certificate for all household members, Alien Registration card(s), amnesty card(s), naturalization papers
SOCIAL SECURITY CARD - Social Security Card or verification from SSA that you applied
EMPLOYEE PAY STUB - Most recent, consecutive 3 month's pay stubs showing the name and address of the employee & employer
OTHER INCOME - Award letter or Statement from Social Security, Retirement(s), Pension(s), VA Benefits, School Grants &/or Student Loan(s), Disability, Unemployment, Passport to Services from Human Service Dept., Child support, AK-PFD
SELF EMPLOYMENT - Last year's tax return, business records for the current and prior year, proof of business expense
ASSET-BANK ACCOUNT(S) - Current savings account statement, AND the last 6 consecutive months of Checking Account statements
